Our VITA program is powered by student volunteers who receive certification in tax training and provide this free service to the community.
DOCUMENTS TO BRING TO YOUR VITA APPOINTMENT
It's important to bring any relevant tax documents to your appointment. For your convenience some types are listed below, but you may have other forms that are not listed. It's helpful if you bring your previous year's tax return, but not required. If you're not sure, bring it anyway.
PROOF OF IDENTIFICATION
Government issued photo ID (Driver's license, state identification card, passport)
Original Social Security Cards (for You, spouse, and ALL dependents)
Bank account (routing and account numbers) for tax payment or refund direct deposit
Previous year taxes
FORMS RELATED TO INCOME
Wage and earning statements from all employers (W-2, 1099-MISC, etc.)
Unemployment statements (1099-G)
Interest statements (1099-INT)
Retirement Income statements (1099-R)
Student Loan Interest Statement (1098-E)
FORMS RELATED TO HEALTHCARE
Affordable Health Care statements (1095-A, B, or C)
Health insurance exemption certificate (if received)
FORMS RELATED TO TAX CREDITS
Amounts paid in tuition and expenses for college (1098-T)
Amounts paid and tax identifiers for daycare providers (name, address, SSN or EIN)
